Check back soon for updated forms and documentation downloads.<\/strong><\/p>\n<h3>Long-Term Disability Insurance<\/h3>\n<p>Campbell University offers long-term disability insurance free of charge to all full-time, eligible employees who have been employed for more than one (1) year. This policy is issued by Voya. Your benefits begin after a waiting period of 180 days at 60% of your Income up to $2,000 per month.\u00a0 This benefit is payable to age 65 or Social Security Normal Retirement Age.\u00a0<\/p>\n<p>To file a claim for long term disability, complete the entire form and return to the <a href=\"https:\/\/www.campbell.edu\/faculty-staff\/human-resources\/about-human-resources\/\">HR office<\/a>.\u00a0<\/p>\n<h3>Voluntary Short-Term Disability<\/h3>\n<p>This policy is issued by Voya.\u00a0<\/p>\n<ul>\n<li>Maximum Benefit Period: allows you to select the duration of coverage.\u00a0 Options are 22 weeks, 24, weeks and 26 weeks &#8211; as long as you are still unable to work due to a covered disability.<\/li>\n<li>Benefit Amount:\u00a060% of your weekly salary, up to $1,500.\u00a0<\/li>\n<li>Injury\/Sickness Start Dates: allows you to select the amount of time that you must be disabled before benefits are available.\n<ul>\n<li>1st day for injury\/8th day for sickness<\/li>\n<li>15th day for injury\/15th day for sickness<\/li>\n<li>31st day for injury\/31st day for sickness<\/li>\n<\/ul>\n<\/li>\n<li>This plan has a 3\/12 Pre-Existing Limitation<\/li>\n<\/ul>\n<h4>Benefit Highlights<\/h4>\n<ul>\n<li>Enrollment is done through Employee Navigator during the open enrollment period or during a qualifying life event.\u00a0<\/li>\n<li><a href=\"https:\/\/presents.voya.com\/EBRC\/CampbellU\">Claim Form<\/a><\/li>\n<\/ul>\n<h2>Workers&#8217; Compensation<\/h2>\n<p>All University employees are covered by Workers\u2019 Compensation Insurance as of their date of employment, for which the University pays the full premium. Under the provisions of the North Carolina Worker\u2019s Compensation Act, as amended, University employees are eligible for certain benefits in the event of a work-related accident resulting in temporary or permanent disability. Unless the work-related injury results in a disability of more than 21 days, the injured employee must use any accrued sick or vacation leave for the first seven (7) days of the period of time the employee is out of work.<\/p>\n<p>When an injury occurs at work, follow the steps below.<\/p>\n<ul>\n<li>If the injury is serious or life-threatening, the employee should:\n<ol>\n<li>Go directly to the nearest emergency room.<\/li>\n<li>Inform his\/her supervisor.<\/li>\n<li>The <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/04\/Workers-Compensation-Injury-Report-Form-04.11.2025-1.xlsx\" target=\"_blank\" rel=\"noopener\">Workers&#8217; Compensation Injury Report Form<\/a> must be completed and returned to Faith McCall (<a href=\"mailto:fmccall@campbell.edu\">fmccall@campbell.edu<\/a>) as soon as possible.\u00a0<\/li>\n<\/ol>\n<\/li>\n<li>If the injury is not life-threatening, the employee should:\n<ol>\n<li>Inform his\/her supervisor.<\/li>\n<li>Make an appointment with Student Health Services.<\/li>\n<li>The <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/04\/Workers-Compensation-Injury-Report-Form-04.11.2025-1.xlsx\" target=\"_blank\" rel=\"noopener\">Workers&#8217; Compensation Injury Report Form<\/a> must be completed and returned to Faith McCall (<a href=\"mailto:fmccall@campbell.edu\">fmccall@campbell.edu<\/a>) as soon as possible.\u00a0<\/li>\n<\/ol>\n<\/li>\n<\/ul>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2016\/12\/21155742\/Employee_Brochure_031612_Campbell.pdf\">If You Are Injured on the Job&#8230;<\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Disability Insurance We have switched to Voya for disability insurance.
