All services to be filed with insurance.<\/span><\/p>\n<h2>Medical Forms<\/h2>\n<p><span style=\"font-weight: 400\">Campbell University uses Magnus Health, a student medical record tracking system. After you have completed your requirements for Admissions, you will receive an email notification through your Campbell email address with instructions on how to complete and submit your health information to Magnus Health.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><b>Note<\/b><span style=\"font-weight: 400\">: As of July 2009, Changes in North Carolina immunization laws now require all individuals enrolling in a college or university for the first time to receive a booster dose of tetanus\/diphtheria\/pertussis (TDaP) vaccine.<\/span><\/li>\n<li style=\"font-weight: 400\"><b>Religious exemption<\/b><span style=\"font-weight: 400\">: Any student whose parents have elected not to immunize against communicable diseases should provide a letter stating your religious convictions and should be signed by the parent or if the student is 18 years of age or older he\/she may sign for themselves.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h2>Copay<\/h2>\n<p>Office visit fee $ 5.00 for BCBS Open Access Plans \u2013 seen by appointment only.<\/p>\n<h2>Available Services<\/h2>\n<p>The following services are available to Campbell faculty and staff:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.campbell.edu\/health-center\/services\/behavioral-health-services\/\">Behavioral Health<\/a><\/li>\n<li><a href=\"https:\/\/www.campbell.edu\/health-center\/services\/family-medicine\/\">Family Medicine<\/a><\/li>\n<li><a href=\"https:\/\/www.campbell.edu\/health-center\/services\/osteopathic-manipulative-medicine-omm\/\">Osteopathic Manipulation Medicine<\/a><\/li>\n<li>Pain Management\u00a0<\/li>\n<li><a href=\"https:\/\/www.campbell.edu\/health-center\/services\/sports-medicine\/\">Sports Medicine<\/a><\/li>\n<li><a href=\"https:\/\/www.campbell.edu\/health-center\/services\/wellness-programs\/\">Wellness Programs<\/a><\/li>\n<\/ul>\n<h2>Accepted Health Insurance<\/h2>\n<p>Your care is our number one priority.
