The following is a list of items that may help you when filling out the form:<\/p>\n<ul>\n<li>Block # 9: DOB \u2013 Date of Birth<\/li>\n<li>Block # 10: POB \u2013 Place of Birth<\/li>\n<li>Block # 24: Next of Kin \u2013\u00a0Who would we contact in an emergency?<\/li>\n<li>Block # 25a \/ 26a: FICE code \u2013 Do not fill this in<\/li>\n<li>Page #2: Initial one box in each section (blocks #42-47) on the second page.<\/li>\n<li>You must sign page #2 where is states &#8220;All information given on this form is correct to the best of my knowledge.&#8221;<\/li>\n<\/ul>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/11\/USACC_FORM_136.pdf\"><strong>CC 136<\/strong> <\/a>\u2013 Government Sponsored Benefits for ROTC Cadets. Sign and date the bottom of the form. This indicates that you have read and understand the information on the form.<\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/11\/USACC_FORM_137.pdf\"><strong>CC 137<\/strong><\/a> \u2013 Authorization for access to student records.<\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05103735\/DD_2983_Trainee_Prohibitive_Activities_ACK.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">DD 2983<\/a><\/strong> \u2013\u00a0Recruit\/Trainee Prohibited Activities Acknowledgment. Complete blocks 1-9 only.<\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/08\/104-R-Planned-Academic-Worksheet-AUGUST-2024.pdf\"><strong>CC 104-R <\/strong><\/a>\u2013\u00a0This form is an academic plan worksheet. It should contain all of the classes you need to complete your bachelor\u2019s degree, as well as, all of the required ROTC classes. You ROTC instructor will advise you on the best method for getting this document completed. <strong>It will require a signature from your academic advisor on page two, block #12. \u00a0<\/strong><\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05103845\/SAL_Sheet.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">SAL Sheet<\/a> <\/strong>\u2013 Scholar Athlete Leader sheet. You only need to provide hair color, eye color, and blood type at the bottom of the form.<\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05103912\/Language_questionnaire.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Language Questionnaire<\/a><\/strong> \u2013 Answer each question\/section that applies to your foreign language abilities.<\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05103940\/PME.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">PME Requirements<\/a><\/strong> \u2013 Read all the PME Requirements and sign the second page saying you understand the requirements<\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2018\/05\/ROTC-Dental_Records-1.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Dental Form<\/a> <\/strong>\u2013 Need to know the location of your dental x-rays \u2013 for emergency purposes only. You are the person that signs this form. ***<strong>Do not bring your dental x-rays.<\/strong><\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/03\/dd93.pdf\"><strong>DD 93 <\/strong><\/a>\u2013 This is a Record of emergency data. Fill out the first two pages; if you need any instruction for how to fill out any part of these forms pages 3 and 4 contain instruction.<\/p>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/03\/2025-SGLV-8286.pdf\">SGLI<\/a>\u00a0<\/strong>\u2013 The Service members\u2019 Group Life Insurance Election and Certificate.<\/p>\n<ul>\n<li>Complete pages 1-3 with required information.<\/li>\n<li>Sign and date page 3<\/li>\n<li>All five pages must be turned in with your contracting packet.\u00a0<\/li>\n<\/ul>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2021\/09\/fast_start_form.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>SF 1199A<\/strong><\/a> \u2013 Direct Deposit form needs to be filled out and turned in in-order to receive direct deposits.\u00a0<\/p>\n<ul>\n<li>Cadet must complete section 1, 2, and 3<\/li>\n<li>Sign and date block 5<\/li>\n<\/ul>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/01\/2025-Form-W-4.pdf\"><strong>W4 <\/strong><\/a>\u2013 Tax form (withholdings: current year)<\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2022\/10\/dd2058_state_of_legal_resident.pdf\"><strong>DD Form 2058<\/strong><\/a> \u2013 State of Legal Residency Form. Fill in name, SSN, and your legal mailing address. Sign and date the bottom<\/p>\n<ul>\n<li>Fill out part 1 if you would want to authorize an ROTC instructor to speak with your parents about your performance, to include grades, should they contact us.<\/li>\n<li>Fill out part 2 if you do not want us to discuss your performance with your parents\/legal guardian.<\/li>\n<\/ul>\n<p><strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05104325\/DA_Form_3425.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">DA form 3425-R<\/a> <\/strong>\u2013 Medical fitness statement. Go to your physician and have that physician fill out this form stating you have no medical conditions or physical impairments that may hinder your participation in ROTC.<\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/10\/DD-Form-2005-JUN-2016.pdf\"><strong>DD Form 2005 <\/strong><\/a>\u2013 Privacy Statement for healthcare Records, Read form and sign and date the bottom.<\/p>\n<p><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2023\/08\/precontracting_checklist_072723.doc\"><strong>Pre-contract Checklist <\/strong><\/a>\u2013 This checklist will be completed in conjunction with your ROTC instructor at the time of contracting; it does not need to be completed prior to contracting.<\/p>\n<p><strong>SMP documents:\u00a0<\/strong>Required for all soldiers currently serving in the National Guard (NG) or Reserves.\u00a0 Also needed if you are considering joining the NG or Reserves in conjunction with the Minuteman or GRFD scholarship application.<\/p>\n<ul>\n<li>Reserve: <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05102454\/DA_4824R_Apr2005.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">DA 4824<\/a>\u00a0and <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05102404\/letter-of-acceptance.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Letter of Acceptance<\/a>\u00a0from Company Commander<\/li>\n<li>National Guard: <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05102052\/NGB-594-1.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">NGB 594-1<\/a><\/li>\n<li>MEPS physical is required for all SMP prospects<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Application Checklist \u2013 This is a checklist of every form needed in order to contract with the ROTC program.
