{"id":6289,"date":"2017-01-05T10:16:33","date_gmt":"2017-01-05T15:16:33","guid":{"rendered":"https:\/\/www.campbell.edu\/?page_id=6289"},"modified":"2025-06-05T12:39:37","modified_gmt":"2025-06-05T16:39:37","slug":"rotc-enrollment-packet","status":"publish","type":"page","link":"https:\/\/www.campbell.edu\/rotc\/recruitment\/rotc-enrollment-packet\/","title":{"rendered":"ROTC Enrollment Packet"},"content":{"rendered":"<p>The eight documents listed below must be completed if the student wants to partake in the extra curricula activities associated with ROTC, such as: Physical fitness training, leadership labs, and Field Training Exercise (FTX) which include rifle ranges, land navigation training, obstacle courses, and leader reaction courses.<\/p>\n<p><strong>1.\u00a0 Proof of Citizenship<\/strong> \u2013 Students must provide official documentation demonstrating that they are a citizen of the United States.\u00a0 This can be completed by producing either a U.S. Birth Certificate or unexpired U.S. Passport.<\/p>\n<p><strong>2.\u00a0 Social Security Card<\/strong> \u2013 Students must provide official document for verification by ROTC Cadre.<\/p>\n<p><strong>3.\u00a0 <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2025\/06\/USACC-Form-139-R-Cadet-Application-and-Enrollment-Record-JUNE-2025.pdf\">CC 139-R<\/a>\u00a0<\/strong>\u2013 (Right click and &#8220;Save As&#8221;) This is a cadet enrollment record form.\u00a0This form is fillable, DO NOT hand-write your information on this form. The form has 6 pages; however, only the first 2 pages need to be completed for contracting purposes.\u00a0The following is a list of items that may help you when filling out the form:<\/p>\n<ul>\n<li>Block # 9: DOB \u2013 Date of Birth<\/li>\n<li>Block # 10: POB \u2013 Place of Birth<\/li>\n<li>Block # 24: Next of Kin \u2013 Who would we contact in an emergency<\/li>\n<li>Block # 25a \/ 26a: FICE code \u2013 Do not fill this in<\/li>\n<li>Page #2: Initial one box in each section (blocks #42-46).<\/li>\n<li>You must sign page #2 where is states\u00a0&#8220;All information given on this form is correct to the best of my knowledge.&#8221;<\/li>\n<\/ul>\n<p><strong>4.\u00a0 <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/11\/USACC_FORM_137.pdf\">CC 137<\/a>\u00a0<\/strong>\u2013 Authorization for access to student records.<\/p>\n<ul>\n<li>Fill out part 1 if you would want to authorize an ROTC instructor to speak with your parents about your performance, to include grades, should they contact us.<\/li>\n<li>Fill out part 2 if you do not want us to discuss your performance with your parents\/legal guardian.<\/li>\n<\/ul>\n<p><strong>5.\u00a0 <\/strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/11\/USACC_FORM_136.pdf\"><strong>CC 136<\/strong><\/a>\u00a0\u2013 Government Sponsored Benefits for ROTC Cadets.\u00a0 Sign and date the bottom of the form.\u00a0 This indicates that you have read and understand the information on the form.<\/p>\n<p><strong>6.\u00a0 <\/strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05104325\/DA_Form_3425.pdf\" target=\"_blank\" rel=\"noopener\"><strong>DA form 3425-R<\/strong><\/a>\u00a0\u2013 Medical fitness statement.\u00a0 Go to your physician and have that physician fill out this form saying you have no medical conditions or physical impairments that may hinder your participation in ROTC.<\/p>\n<p><strong>7.\u00a0 <a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2017\/01\/05103735\/DD_2983_Trainee_Prohibitive_Activities_ACK.pdf\" target=\"_blank\" rel=\"noopener\">DD 2983<\/a>\u00a0<\/strong>\u2013\u00a0Recruit\/Trainee Prohibited Activities Acknowledgment. Complete blocks 1-9 only.<\/p>\n<p><strong>8. <\/strong><a href=\"https:\/\/assets.campbell.edu\/wp-content\/uploads\/2024\/08\/104-R-Planned-Academic-Worksheet-AUGUST-2024.pdf\"><strong>CC 104-R\u00a0<\/strong><\/a>\u2013\u00a0This form is an academic plan worksheet. It should contain all of the classes you need to complete your bachelor\u2019s degree, as well as, all of the required ROTC classes. You ROTC instructor will advise you on the best method for getting this document completed.\u00a0<strong>It will require a signature from your academic advisor on page two, block #12.
