Family Educational Rights and Privacy Act (FERPA)\u00a0<\/span><\/h3>\n

What is FERPA?<\/strong><\/h4>\n

The Family Educational Rights and Privacy Act of 1974, 20 U.S.C. \u00a7 1232g (FERPA) is a Federal law that protects the confidentiality of student information.\u00a0 More specifically, FERPA protects personally identifiable information in students\u2019 education records from disclosure without written consent unless an exception to the consent requirement applies.<\/p>\n

Identity and Statement of Educational Purpose\u00a0<\/span><\/h3>\n

2025-2026 Identity and Statement of Educational Purpose Form – For Notary\u00a0<\/a><\/p>\n

2025-2026 Identity and Statement of Educational Purpose Form – In Person<\/a><\/p>\n

The Identity & Statement of Educational Purpose <\/span>must be notarized or completed in person<\/b>. <\/span>Notarized Identity & Statement of Educational Purpose forms require the original notary form to be mailed to the Office of Financial Aid with a copy of a photo ID. Visit the<\/span> Office of Financial Aid<\/span><\/a> to complete on campus.\u00a0<\/span><\/p>\n

If you are unable to appear in person, the Notary Identity and Statement of Educational Purpose form can be downloaded from your Verification Gateway task list if required.\u00a0 Please visit Verification of FAFSA to learn more.\u00a0<\/a><\/p>\n

NOTE: Only complete the Identity and Statement of Educational Purpose if it has been requested by the Office of Financial Aid or Verification Gateway.<\/strong><\/p>\n
